PURPOSE
Responsible for evaluation, treatment planning, referral, care management, individual follow-up, and group therapy based on clinical evaluations of targeted populations.
Acts as a consultant and liaison to physicians and clinical staff as well as outside agencies.

ACCOUNTABILITIES
1) Provides mental health assessments and individual and group psychotherapy in order to implement goal specific and measurable treatment plans.
2) Screens and evaluates patients for referral to an appropriate mental health provider.
3) Provides urgent care and crisis intervention as indicated.
4) Provides more specialized treatment modalities where qualified by special training.
5) Consults regularly with psychiatrists on patient care.
Functions as resource for center providers who are working with patients with psycho-social problems.
6) Communicates with Behavioral Health colleagues, Primary Care providers, and other Kaiser Permanente providers and agencies as needed to facilitate integrative care.
7) Counsels and refers patients to community resources as appropriate.
Assists in maintaining liaison between community health, welfare agencies, and other outside agencies.
8) Documents clinical patient encounters in accordance with department policies and procedures following
HIPAA
regulations.
9) Assists in implementing in-service education and support programs for medical center staff.
10) Prepares and submits reports on mental health activities as requested.
11) Participates in staff development meetings and in quality of care activities on a regular basis.
12) Manages panel and practice management within departmental guidelines.
13) Performs other related duties as assigned.
Minimum Requirement - Relevant Years of Experience:
